To comply with TIC regulations, Anthem provides Machine Readable Files (MRFs) showing in-network negotiated rates and out-of-network allowed amounts. To access the City of Cincinnati’s MRFs, visit Anthem’s website and enter the City’s Employer Identification Number (EIN) in the search box.

Due to recent IRS changes, IRS Form 1095 will only be provided upon request, as it is no longer needed to file taxes or for other official purposes.

Employees who would like a copy of their 1095 should complete the below form to have one provided.
